CONSENT TO RECEIVE NOTICES VIA EMAIL

I am an homeowner of Villages at Country View, Monroe Twp and thereby a member of Villages at Country View Condominium Association, Inc. (the “Association”). The Association is required to send written notices to all homeowners regarding Association business in writing via the U.S. Mail. This requirement is set forth in the Association's Master Deed and Bylaws. I agree to the following:

1. I agree to provide my current email address to the Association, and I consent to receive all notices from the Association through my email address in lieu of receiving a written notice delivered through the U.S. Mail.

2. I understand that it is my responsibility to advise the Association's Community Manager if my email address changes and to provide the correct address to the Community Manager.

3. I am providing my email address to the Association for Association business and notices only. I request that the Association hold my email address with confidentiality and that it not use my email address without my authorization.

I, the undersigned, HAVE READ AND UNDERSTAND the terms set forth above, and I am signing this document freely and of my own accord.
