Visitor Parking Passes will only be released to the owner or authorized persons listed below with a valid photo ID

I, the owner of the above-listed property, give authorization for the person listed above to pick up my visitor parking passes from the HOA Management Company. I understand that each property will receive two (2) passes at no cost to the owner. In the event that a visitor parking pass is damaged or lost, I am responsible for paying for the replacement pass(es). The cost of replacement passes will be $25 each. I understand that it is my responsibility to transfer tags between any tenants. In the event that a visitor parking pass is lost or damaged, I will report the lost pass(es) to the HOA Management Company immediately.
