I/We, and of , being a member/members in good standing of Shepherd Hills Homeowners’ Association, do hereby grant my/our proxy for the sole purpose of voting at the 2025 Annual Meeting as follows (check one (1) only):
This proxy is executed for the 2025 Annual Meeting scheduled for May 28, 2025, at 8:00 p.m. or any adjournments or continuations thereof. If you check B, but the name of the proxy holder is left blank, it will be assigned to the President. If A, B, and C above are left blank, this proxy will be used for quorum purposes only. This proxy must be signed and dated below in order to be valid.
PROXY INSTRUCTIONS
- A proxy may be revoked only by written notice from the owner/owners to the person presiding at the meeting.
- Print your name and address where indicated at the top of the proxy.
- Check box A, B, or C (only 1 box). If you check box B, print the name of the natural person you designate to be your proxy holder.
- Sign and date the proxy.
- Any proxy shall be void if it is not signed and dated, if it purports to be revocable without notice, or if the signature of any of those executing the proxy is invalid.
