If you are requesting a replacement/reissued sticker, there is a $25.00 replacement fee per sticker. For fastest processing, please follow the instructions below to submit your payment before completing this form. This form will not be processed if payment has not been made.

- Once logged in, click 'Make Payment' to access ClickPay.
- As the charge may not yet appear on your account, select the 'Edit' button next to the balance, then select 'Alternative Amount' to manually enter the device charge.
