SPECIAL MEETING – Monday, August 4, 2025; 6:30 p.m.

PROXY

The Parks of Carolina Forest Community Association, Inc. (hereinafter, “Association”) Special Meeting will be held on Monday, August 4, 2025 at 6:30 p.m. via teleconference. This Proxy is to be filled out by every homeowner of the Association, in its entirety, and signed at the bottom.  Please fill out all sections, and return to FirstService Residential no later than  Thursday, July 31, 2025.

Whether you DO or DO NOT plan to attend the Special Meeting, please fill out this form and return it so your vote can be cast.

SECTION A – Proposed Amendment (CC&R’s): Commercial Vehicles

SECTION B - ATTENDANCE:  Please indicate whether you plan to be in attendance on the teleconference.

SECTION C - PROXY AGENT:  The undersigned member of The Parks of Carolina Forest Association, Inc. (hereinafter, “Association”) hereby appoints Elizabeth Guerrier (secretary), to act as my Proxy Agent to vote on my behalf as he or she deems fit on any matter that may come before this Special Meeting, except for the items included in the Proxy, and directs said Proxy Agent to vote as indicated with regard to each Section at the Special Meeting and any adjournments thereof.  The Proxy grant may be withdrawn at any time by the undersigned, in person or in writing, prior to the commencement of the meeting. Please note the Proxy Agent must be in attendance at the Meeting.
