The undersigned, as a member of the Park West Homeowners Association of Myrtle Beach, Inc., hereby appoints Mike Pavlick, Secretary, or (Proxy holder must be present at meeting) with full power of substitution, as attorney in fact and proxy for the undersigned with respect to the vote which I (we) am entitled to cast at the annual meeting of the Park West Homeowners Association of Myrtle Beach, Inc., on Monday, April 27, 2026, or any adjournment or change thereof. This appointment as attorney in fact and proxy shall be for any and all business that shall properly come before the meeting, including the election of Directors, and does specifically revoke any and all attorneys in fact or proxies previously executed by me (us) for this meeting.
This Proxy may be revoked by the undersigned at any time (provided I give notice of revocation to the Vice-President of the Association) and it shall be deemed to be automatically revoked if the undersigned, personally attends the Annual Meeting.
SECTION 1 – ELECTION OF DIRECTORS: Vote for up to two (2) candidates by placing a check mark before the name of the candidate(s). You may write in a candidate's name but no more than two (2) names total may be selected.
Applications for each of the candidates listed above are included in your Annual Meeting Packet.
SECTION 2 - ATTENDANCE: Please indicate whether you plan to be in attendance so we can properly plan for the meeting. A quorum of 51% is required to hold the official meeting.
SECTION 3 - CONSIDERATION OF TAX RESOLUTION: Resolved, that any excess of Association income over Association expenses are defined in IRS Reg. 1-277-1 for the year ended December 31, 2026, shall be applied against the subsequent tax year member assessments as provided by the IRS Revenue Ruling 70-604.
