pursuant to Article II,   Section 10 of the By-Laws of Fort Ellsworth Condominium Apartments Unit Owners' Association for the purpose of establishing a quorum and casting my/our votes as instructed below at the Association Meeting on April 16 or any recess or adjournment of the meeting.

If the proxy designation is left blank, I/we grant my/our proxy to the Secretary of the Meeting. If no votes are designated, the Form shall be used for quorum purposes only.

My/our proxy is directed to cast my/our vote for the following candidates to serve on the Association Board of Directors. (You may vote for up to four (4) candidates. If you select more than four (4), your vote for election matters will not be counted.
