IMPORTANT: Tenants must attach a copy of their lease or lease extension letter to this form. Only tenants who are expressly named in the lease will receive parking stickers. Within the lease, tenants must also agree to abide by the rules and regulations of Carriage Park. Incomplete leases will result in parking sticker delays.
NOTE: If you are replacing a vehicle, please put the new vehicle information under "Vehicle 1" and the old vehicle information under "Remove This Vehicle from Account" and check the "Replacing This Vehicle" box. To avoid paying a $25 replacement fee, please keep your previous sticker.

If any of the above information changes, the Resident is required to inform the Association at the address listed above. If you are an owner and selling your unit, please inform the buyer of the need to register their vehicles and be sure to supply your visitor hang tags to the new owners.

There will be a $25 charge to replace a lost resident decal or visitor hang tag. Please pay for the replacements online through your resident portal using the instructions below. If you are not requesting a replacement, do not make a payment.
Instructions for making resident portal access device payments: Login to your portal at https://CarriageParkCondominium.connectresident.com. Once logged in, click 'Make A Payment' to access ClickPay. As the charge may not yet appear on your account, select the 'Edit' button next to the balance, then select 'Alternative Amount' to manually enter the payment amount. See the attached payment screenshot example as well. If you have trouble making a payment through the resident portal, please call 24/7 Customer Care Center at 1.800.870.0010 so they can walk you through it step by step over the phone.
To request a temporary visitor pass or passes for special events, email [email protected]. Please give at least 48 hours notice to allow for processing by a particular date.
