REQUEST FOR HOMEOWNER INSURANCE

To ensure that all homes within our community are properly insured and to assist the Association in maintaining accurate records, we are requesting that all homeowners submit a copy of their current Homeowners Insurance Declaration Page showing coverage under an HO-3 or HO-5 policy.

The Association's policy does not cover:

• Your personal townhome
• Your personal belongings
• Your personal liability

This applies whether you own or rent your home.

Please attach your HO-3 or HO-5 homeowners insurance policy.
