owner/owners of hereby designate the following as my/our proxy holder, pursuant to Article III, Section 6 of the Blooms Mill Homeowners Association, Inc. Bylaws, for the purposes of voting and establishing quorum at the 2025 Annual Meeting and any recess, adjournment or subsequent meeting called if the required quorum is not present. 

My/our proxy shall vote on directors as indicated below and shall vote on my/our behalf for any other matters that come up for a vote at the meeting. (Your proxy holder must attend the meeting in order for your vote to count.)

Election of Directors. Three positions on the Board are available. You must select no more than three candidates. If more than three candidates are selected, your proxy will be counted for quorum only.

Vote for NO MORE THAN THREE:

If no boxes are checked, the proxy form will be counted for quorum purposes only.
