Avonlea HOA Pool Fob Request Form

Please complete the form below to request a pool fob. Each household is permitted to purchase one fob. The cost of each fob is $50.

Important Information:
This fob will replace any previously issued fob for the property listed above, as only one active fob is permitted per household.

Fobs may only be purchased by the lot owner. Tenants must contact the lot owner directly to coordinate the purchase and submission of this request.

If a fob is lost or damaged, a replacement may be issued for a $50 replacement fee, and the previous fob will be permanently deactivated. The HOA is not responsible for any access issues resulting from lost or damaged fobs.

Acknowledgement
By my digital signature below, I acknowledge that I am the owner of the above property and understand that the cost of the fob is $50. I further acknowledge that only one fob is permitted per household and agree to comply with all pool rules and regulations. I understand that once the new fob is activated, any previous fobs will be deactivated.
