2025 Annual Meeting

Please use this form only if you do not wish to vote in person at the Annual Meeting of the membership on October 30, 2025. If you have any questions regarding this form, please contact the Association’s management or a member of the Board of Directors.
Part I - Appointment of Proxy:
Print the name of your proxy, or, if left blank, the Association Secretary will be designated as your proxy. Please note that the Association’s Secretary will only cast an instructed vote or designate you as present for quorum purposes only (options B and C below). The Secretary will not cast an uninstructed ballot as your proxy (option A). If you complete this form, but fail to check either of box A, B or C below, you will be designated as present for quorum purposes only.
The undersigned hereby appoints as my proxy to represent me/us and to vote on my/our behalf as instructed on any and all matters which may come before the Members at the annual meeting of the Avonlea Homeowners Association on the 30th day of October 2025, at 6:30 p.m., and at any continuation or adjournment(s) thereof. I instruct my proxy to vote as follows:

The person named in this proxy may cast (my) (our) votes on any issue arising during the meeting for vote by the members as he or she sees fit. THIS MEANS THAT THE PERSON APPOINTED AS YOUR PROXY MAY USE THEIR DISCRETION TO VOTE ON ANY MATTER BEFORE THE MEMBERSHIP FOR VOTE DURING THE SPECIAL MEETING.
